Position Summary:
Health Referral and Liaison Officers (HRLO) will play a critical role in ensuring functional two-way referral systems between communities, clinical services, and social service providers. Experience in facilitating partnerships with community-maintained facilities and experience in referral systems is preferred.

Key duties and responsibilities

Provide case management and follow-up for all children identified as HIV + in selected districts.

Provide case management and follow-up for all GCBS grantees prioritizing:

CALHIV, including access to ART and viral load monitoring

Access to PREP for 10% of GCBS adolescents (15-17 years)

Child HIV + Caregiver Index Tests

Teen mothers to access CPS

Access to interventions related to gender-based violence and sexual and reproductive health

Ensure functional two-way bypass systems between DOH facilities, DSD service points, and NPOs in selected districts.

Provide direct feedback and support to ensure the success of referral systems and provide support to troubleshoot and resolve issues as needed.

Get involved with HIV testing and counseling providers and do community outreach.

Track incomplete referrals by phone or face-to-face from SSPs and the health center.

Co-facilitate the formation of support groups for dissemination and adherence in health centers.

Provide monthly feedback on the number of referrals of HIV + children to DSD staff, NPO staff, GCBS staff, and health center staff meetings.

Work together and support data support offices to compile your monthly statistical report using the GCBS data collection tool with an emphasis on CALHIV case management.

Report monthly on PEPFAR’s referral systems and key performance indicators to PPM or the District Health / Nurse Referral and Linkage Specialist.

Participate in DOH district coordination meetings and forums.

Actively participate in district and site level partner (NPO) meetings that support the achievement of program goals, including those facilitated through DSD, DOH, district support partners (DSP), other OVC partners funded by PEPFAR and any local forum.

Promote and maintain teamwork and open communication for a motivated workforce at all project sites.

Education and experience is required:

Designated Auxiliary Social Worker (SAW), Child and Youth Care Worker (CYCW), or Community Worker.

At least three years of experience in a field position with an organization in public health or orphaned and vulnerable child programming; Experience in the development and management of referral systems is preferred.

Experience with PEPFAR funded projects in South Africa.

Able and willing to work flexible hours as needed and without constant or close supervision.

Driver’s license
